ENROLLMENT GUIDELINES AND PROCEDURES

Initial Requirements for Online System or Physical Enrollment Processing

     In case that enrollees have not yet claimed official documents from their respective previous schools, the following initial document/s may be attached in the online registration form (for online system) or presented and submitted to the Registrar (for physical enrollment processing) for temporary enrollment.      
           
For Basic Education including Senior High School:

  1. Scanned, digital or screenshot copy of most recent Report Card (SF 9)
  2. Scanned, digital or screenshot copy of Recommendation Letter or Certificate of Good Moral Character signed by the Principal or Guidance Counselor from previous school (if readily available).

For Incoming College Freshmen

  1. Scanned, digital or screenshot copy of most recent Report Card (SF 9) and/or copy of grades.
  • Scanned, digital or screenshot copy of Recommendation Letter or Certificate of Good Moral Character signed by the Principal or Guidance Counselor from previous school (if readily applicable).

 

For New Graduate Students

  1. Scanned, digital or screen shot copy of Official Transcript of Records from previous school for pre-assessment

For Graduate Transferees

  1. Scanned, digital or screen shot copy of Official Transcript of Records for validation and subject crediting (as per crediting guidelines)

Compliance of Complete Requirements

Within four (4) weeks from the start of physical classes, the following requirements must be submitted to the class adviser (for Basic Education and SHS-DPRO), Program Chairs (for college) and Graduate School Dean (for graduate students) upon preliminary verification, who shall endorse them to the Registrar for final verification. Failure of the student to comply and/or submit the said requirements within the prescribed period may result to cancellation of his/her enrollment.

A. BASIC EDUCATION

  • New students and transferees

1.   Original and photocopy of Report Card (SF 9)
2.    Recommendation Letter or Certificate of Good Moral duly signed by
       the Principal or Guidance Counselor from previous school

  1. Two (2) photocopies of Philippine Statistics Authority (PSA) Birth Certificate
  2. Two (2) photocopies of Baptismal Certificate
  3. Two (2) pcs. long folders with fasteners
  4. Four (4) pcs. 1×1 ID pictures
  • Foreign students
  1. Academic documents authenticated by the Philippine Embassy and/or Consulate official located in the student’s country of origin (with English translation, if written in foreign language)
  2. Student’s Personal History Statement
  3. Photocopy of student’s passport showing biopage, latest arrival and authorized stay in the Philippines
  4. Original and photocopy of Birth Certificate
  5. Three (3) pcs. 2×2 ID picture
  6. Three (3) pcs. 1×1 ID picture
  7. Two (2) pcs. long folders with fasteners

B. COLLEGE

  • Freshmen

PAYMENT

1. A minimum downpayment of P1,000.00 may enable a student to enroll to his/her respective grade/year level or any of his/her preferred course program/track and strand. This amount may be deposited to the bank thru the indicated Acct. Numbers (for online enrollment system option) or paid directly to the cashier (for physical processing enrollment option). You may deposit payments thru Olivarez College bank accounts enlisted below:

 

2. Payments can also be transacted thru pay counters at Olivarez College Accounting Office.

3. Students may also pay the amount based on their preferred payment scheme.

5. Deposit slip and/or transaction receipt shall be attached to the online enrollment registration form, or presented and submitted to the Accounting Office at Olivarez College.

6. Old students with pending balances can call the Accounting Office at 88260750 loc. 101 /88258712 prior to enrollment.

7. Students must keep the official receipt issued by the school.

8. To note, payments shall only be transacted directly to the Accounting Office and not with any person, middle man or any of the Olivarez College employees, other than the cashier nor to any bank entities, other than those official accounts.

10. Official receipts of those, who processed enrollment thru online system, shall be issued upon coming to Accounting Office. However, those who will enroll using physical enrollment processing option, receipts will be immediately released and issued to them. Parents may claim their children’s receipts directly from the Accounting Office anytime upon payment.

 
ENROLLMENT PROCESSING PROCEDURES

There are two (2) enrollment process options: (1) the online enrollment system; and (2) the physical enrollment processing. The following are the procedures:

  • ONLINE PROCESSING SYSTEM    
Go to Olivarez College official website. Those, with remaining balance on their accounts, may settle it first or arrange it with the Accounting Office, thru the following official contact numbers: 88260750 loc. 101 /88258712.
Click “Online Enrollment” tab at the upper right  corner.
Choose and click your department (Grade School Department / Senior High School Department /College Department /Graduate School Department).
Choose and click your status (New / Old Student)
Click “”Continue” button
If you select NEW STUDENT the NEW STUDENT REGISTRATION will appear.
Accomplish the student registration form. (Fill-out the form with all data/information needed.)
If you select OLD STUDENT the OLD STUDENT ENROLLMENT will appear
Just type your STUDENT NUMBER and LAST NAME, then Click SEARCH.

Select bank or any payment center then type the Transaction Number and the Amount of the payment. You must scan or screenshot the bank deposit upload the file (less than 1MB in size) and click UPLOAD NOW.

For DPRO/Public SHS Grade 11 and 12 .    

1. Choose ANNUAL as your mode of payment
2. Payment thru Department of Education
3. Fill in the transaction number with your LRN.
4. For the amount, just type in 1.

   
Once you are done with all the needed information, review the data and then, click Save Information Now button.
Upload preliminary required documents (for temporary enrollment).
Upload Screenshot or Photo of your receipt
After you upload, click button UPLOAD NOW. Another button will appear. Click CLICK THIS BUTTON TO GET YOUR REFERENCE NUMBER button..
After you click the button, a temporary assessment slip will appear on your screen which confirms that your registration is succesful.
 You may print, save, or screen capture the said confirmation notice/slip to serve as your temporary student pass on the first day of class.
PHYSICAL ENROLLMENT PROCESSING SYSTEM
This is the usual manner of enrollment done directly at the school.

Basic Education

  1. Submit the credentials to the Registrar’s Office. In case that these are not issued yet by your previous school, initial requirements are accepted, such as photocopy, scanned, screen shot copy of most recent report card and certificate of good moral conduct or recommendation from previous school (if readily available).
  2. Fill-out the application form.
  3. Pay the assessed fees at the Accounting Office.
  4. Present receipt to the Registrar’s Office.
  5. Class schedule will be given on the first day of classes.

College

  1. Submit the credentials to the Registrar’s Office. In case that these are not issued yet, initial requirements are accepted, such as photocopy, scanned, screen shot copy of most recent report card, and certificate of good moral conduct or recommendation from previous school (if readily available). (For Olivarez College Senior High School graduates, endorsement by the Principal and Asst. Principals may serve as initial document.) Students may directly enroll on their preferred course programs anytime.
  2. Fill-out the application form.
  3. Copy the class schedule and have it validated by the Registrar.
  4. Pay the assessed fees at the Accounting Office.
  5. Present receipt to the Registrar’s Office.
  6. Get your official registration form (student’s copy) that will serve as temporary admission pass to be used on the first day of classes.

Graduate School

  1. Submit the credentials to the Registrar’s Office. In case that these are not issued yet, initial requirements are accepted, such as photocopy or scanned copy of transcript of records (TOR) to be assessed and validated by the Registrar.
  2. For transferees, the copy of TOR shall be assessed by the Dean of Graduate School for crediting and validation purposes as per validation guidelines.
  3. Fill-out the application form.
  4. Copy the class schedule and have it validated by the Registrar.
  • Pay the assessed fees at the Accounting Office.
  • Present receipt to the Registrar’s Office.
  • Get your official registration form (student’s copy) that will serve as temporary admission pass to be used on the first day of class/es.

For College Irregular Students, Transferees and Returnees

            Proceed to the respective departments for academic advising to prescribe subjects and necessary arrangements based on your individual course curriculum plan, hence, you will go through physical enrollment processing at designated Registrar’s counter. Then, follow the aforementioned physical enrollment processing procedures.

For Foreign Students

Foreign students need to personally visit the Registrar’s Office for verification of government requirements and application. Then, the assigned registrar’s staff will assist you in proceeding to the next steps, following the aforementioned physical enrollment processing procedures.

 

POST-ENROLLMENT PROTOCOLS

            In order to adhere to government health protocols and ensure safety and protection of students, the following guidelines for activities are set by the institution for the coming class opening.

A. Submission of Required Documents

            Student’s temporary enrollment will be converted to official enrollment once he/she has presented and submitted the required admission documents enlisted above. The said documents shall be submitted during class meeting to the class adviser (for Basic Education and SHS-DPRO) and Program Chairs (for college) and Graduate School Dean (for graduate students), who shall endorse them to the Registrar upon preliminary verification. The Registrar will then, do the final verification of said documents. If there are concerns with such, the Registrar will call the attention of the concerned students  thru the Principal, Asst. Principals or Program Chairs, to explain and clarify the relevant concerns or deficiencies and will be given time to fix or comply them. To note, parents of little kids (preschool to grade school pupils) should be the ones to process and transact the documents with the school, thru the respective departments.
 
B. ID Processing

            Official school ID will be processed within weeks after school opening as per schedule. Safety guidelines will be imposed by the school and oriented to parents/guardians.

C. Gate Admission

            For security and safety, new students, transferees and returnees shall present the enrollment confirmation notice or slip on the first day of classes to the guards on duty upon inspection for entry. The said confirmation notice will be replaced by student admission pass to be issued by the school thru the respective departments and class advisers. This will serve as temporary student pass until such time that official IDs have been printed and issued to students. Old students need to wear prescribed official ID upon entry to school premises.

D. Orientation Procedures

           Parent and Student Orientation Sessions will be arranged and scheduled by the institution, thru the respective departments. In order to abide by the “No Mass Gathering” protocol imposed by the government, it may be done through virtual videos, while official printed policies in a newsletter form will be sent to parents thru their email addresses. The schedules of orientation will be posted in the official school website or sent thru their email addresses.

E. Administering of Diagnostic Test

           Diagnostic test will be administered to students as per schedule. Such schedule  will be communicated to students thru online (OC website, SMS, group chat, google account, etc.)

F. Club/Student Organization Formation
            As has been a tradition, club formation will still be conducted but this time, thru online or virtual system with process and procedural guidelines to be disseminated by the Office of the Student Affairs. 

G. Guidance and Counseling Service

            The institution will be providing guidance and counseling services and consultation sessions with students who need to process their emotional, academic and personal concerns. This can be done through virtual and online conversation upon request addressed to the Guidance and Counseling Office in its official google account. 

H. Immediate Matters and Concerns for Quick Response

            Each office has a suggestion and concern box for matters that demand immediate and quick response. In addition, concerned students and/or parents may also message their concerns and/or inquiries to respective offices and departments through the official google accounts. In this case, respect and courtesy are expected all the time.
